2024-2025 Registration Form

 

Please fill out a registration form below so we have emergency contact, medical, and other information to help us communicate with parents.  The form is for all Sunday and Wednesday programming for 3 year olds through 12th grade.

 

PAY REGISTRATION FEES for Sunday School (Age 3- Grade 5) of $30 per child or Confirmation (Grades 6-9) of $50 per student. No fee for Youth Group. Once you fill out the online form below, you will be directed to the payment page- or click HERE to pay, or bring in a check to church.
